The top includes an area to record the date and time of the notarization followed by information blocks where you can record the personal information of the signers from the identification they provide. You may then list each document requiring notarization on the lines including the document date, the type of notarization and the description of the document being notarized. In the boxes next to the description of the document, you can instruct the signers to initial each box for every document they sign to indicate they personally acknowledge signing each document. Any additional document lines can be crossed out or designated as having no entries. If your state requires thumbprints, this can be captured at the bottom of the page next to the Notary Notes section. This note section at the bottom also includes an area to record your fee, the miles traveled and any other pertinent information such as special circumstances or signatures of witnesses.If your transaction has more documents than space allows, you may check the box on the bottom left of the page that says "Continued on Next Page" and continue listing your transactions on the next page.
